Each scholarship will be a one-time $500 scholarship.
Applicant Qualifications:
- Applicant must be a high school senior.
- Applicant must be planning on pursuing further education including: two-year college, four-year college, trade school, certification program, etc.
- Applicant must be a member of a walk team for the 2009 Pink Ribbon Walk to be held on May 17, 2009.
- Students will be selected on the basis of eligibility, essays and leadership skills. Grades, test scores and financial need may also be considered.

- All questions must be answered.
- You must use the form provided to respond. All answers and essays must be typed in the space provided. You may paste your essay from your word processor. Do not exceed the space provided.
- Please supply three (3) reference letters with the completed application. At least one of the letters should be from an academic source and one from a personal source (non-family member).
- Please read carefully and sign the final page with all required signatures.
- Forward completed application and three references to your school counselor.
School Counselor Instructions:
- Please complete “School Counselor Page”.
- Attach transcript of grades to application.
- Mail completed application and correspondence to the address below.
Your application will not be returned, please make copies for your own records. Notification of awards to applicants will be made on May 17, 2009 at The Fourth Annual Pink Ribbon Walk at NUS Consulting in Park Ridge, NJ. Applicants need to be present to win. In addition, written notification will also be sent. Please do not call the office regarding applications.
